5 Tips for Successfully Filing a Reimbursement Claim

A health care or dependent care Flexible Spending Account (FSA) is a great way to save money on everyday health care expenses and services and dependent day care. While the savings are always great, the occasional paperwork can sometimes be a hassle, especially when clerical issues delay reimbursement.

Helping to make sure that your claims are approved the first time, every time, is one of our biggest priorities. That is why we continually improve the process and develop new technologies like the online claim form and the EZ Receipts ™ mobile application to simplify the claim process.

The result of our efforts is a set of best practices, used daily by our Customer Service team to instruct participants on our claims submission process. We’ve reproduced it here to help ensure that your claims are reimbursed quickly and accurately.
  1. Always double check your documents to make sure they are complete and clearly legible. The number one reason for a claim being denied is because of illegible or incomplete documentation.
    • Check that it isn't too dark or light and that information is not cut off
    • Do not use highlighters on documentation
    • Make sure handwritten documentation includes stamped provider information
  2. Make sure that your documents include the following five pieces of information required by the IRS. If they don’t, your claim will be denied.
    • Provider's Name
    • Service Date(s)
    • Patient's Name (Requests for over-the-counter (OTC) drugs do not require the patient's name, but do require a prescription)
    • Type of Service (The "bag tag" works for prescription claims)
    • Amount Billed
  3. If possible, use an Explanation of Benefits (EOB) statement from your insurance carrier as the primary documentation, especially if your insurance paid a portion of the expense.
  4. Do not send carbon copies or cancelled checks; they typically do not include all five key pieces of information—and be sure credit card receipts contain all five information pieces listed above.
